When notifying the NTSB, you must provide:

Make, model, and serial number of aircraft.

Precise geographical location or airport name.

Full names and contact information.

Departure and intended arrival airports.
Nature of flight (e.g., personal, instructional, commercial).

A concise, factual description of the accident or incident.
Number of people onboard, including crew and passengers.
Number of fatalities or serious injuries.
Nature and extent of aircraft damage.
